There are two steps, both are interactive on line steps. 

Contact Names  Step 1
First is a contact list - this will  be used to contact  people, that will have access to your building - keys.  Name of the building, Address, Phones, and up to 3 people names and their best phones.  

Shelter ID number will be YOUR phone number 4199462811
Fill in each block, once your done click the OK button at the bottom.

Detail Information Form  Step 2
This a simple fill in the blocks document form,  click on the link, download the shelterform.doc    Use the SAVE AS button - use your phone number for the file name.    4199462811.doc  Complete as much as YOU can on each page.

Once completed then click SAVE, then  e mail the file as an attachment to
